Im interested to know who is performing TEE guided transcatheter interventions (LAAC, Mitral, Tricuspid, ASD/PFO) in your institutions (anesthesia vs cardiology). If done by cardiologist – who is placing the TEE probe (anesthesia vs cardiology).

At our hospital we have a hybrid model.  5/6 interventional echocardiographers are cardiologists.  1/6 is a cardiac anesthesiologist.  All imagers have advanced echo training.  The imaging doc inserts the TEE probe, which is documented in the structural report.

Cardiology started pushing for probe insertion for interventional TEE so we can get our complete RVU since 1/3 is inculved in probe insertion. Need to make sure to document in our report.

In my institution, interventional TEE was performed by a cardiologist that has advanced echocardiography certification.

Both specialties are well capable of doing probe insertions, but mostly anesthesiologist did the insertion. We work as a team.

In most cases , a dedicated cardiologist does the TEE for mitral clips and LAA
Anesthesia places the probes 60 % of time- especially if difficulty is encountered.

Anesthesia ( myself ) does TEE  for TAVRs done under GA and for Watchman's. I have done TEE for ASD device closures in children.

both cardiology and anesthesia are involved - when cardiology is doing the TEE for that day, cardiology places the probe. we share the days with cardiac anesthesia. sometimes, anesthesia helps us to place the probe when airway is small.
